Our nurse-led team comprises well experienced, competent and knowledgeable individuals who are committed to ensuring that our service users are provided with bespoke and personalised care that is of high quality and enriching to their lives. We maintain a quality service provision by following regular audit processes and management controls to ensure the highest standard of care delivery for our service users.
We manage a range of complex care needs for children, young people and adults who require care in:
Tracheostomy management, Home Ventilation management, Head injury, Spinal injury, Neuro-degenerative conditions, Challenging behaviours, Hereditary conditions, Learning disability support, End of life care.
The assessment and care implementation process:
We conduct initial assessments for service users, cooperating with providers, multidisciplinary teams, and clients to develop a plan that centres around the patient from the initial assessments and ensures the service user goals are met by involving them in the process. Regular reviews of these care plans are done to ensure up to date care and ensure the high standards are consistently maintained.
The entire assessment process involves the client and the family to provide a person-centred care service that considers their needs and goals. A multidisciplinary approach ensures that care is implemented to involve all relevant health professionals while ensuring that our service users are at the centre of everything we do.
Reviewing the service regularly, our team of nurses identifies training needs related to the service user’s needs. Detailed interviews are then conducted with staff, their clinical competency and training needs are assessed and addressed prior to introducing staff to service users.
Complex Care Training Academy
Tezlom provides an in-depth training programme for all our staff, equipping them with the correct skills necessary to meet our service users’ needs and conditions. Our team have their training needs assessed before they begin a comprehensive induction program in line with the Care Certificate and include but not limited to:
Medication management, Seizure management, Gastrostomy care and Enteral feeding, Tracheostomy care and Ventilation care.
We also work in partnership with specialist providers, giving our staff access to a wide range of training opportunities that cater for the different conditions and needs they may encounter.
We always make sure our staff are trained to the highest standards and maintain regular competency checks, ensuring their knowledge and practical skills are reviewed as part of ongoing Continuing Professional development.
Our complex care staff team receives regular planned supervisions and spot checks for competency and best practice reassessment. This provides support for the care staff and allows them to reassess their progress and give feedback. Staff appraisals are completed regularly so we can ensure our staff are always well supported, meaning they can deliver our service users with a compassionate, dignified and respectful care service for the different conditions and their needs.
